Video Summary
Carrie Langstraat reviews repair of an intraoperative bladder injury. Expose the full defect, assess its extent and location, and identify the ureters by inspection, IV-administered dye, or stents. Mobilize the bladder as needed for a tension-free repair, then close it in two layers with fine absorbable suture: the bladder wall followed by an imbricating seromuscular layer. Perform a leak test by filling the bladder, and leave a catheter for one to four weeks, based on the patient’s circumstances. In the illustrated case, prior radiation had thickened the bladder; an interpositional tissue flap may help support closure in irradiated patients.
