Video Summary
Ahmed El-Niami demonstrates diaphragm resection in two stages: liver mobilization and diaphragm dissection. The falciform and right triangular ligaments are divided, and traction-countertraction with sharp dissection and energy separates the liver from the diaphragm. Further mobilization exposes Morrison’s pouch and the liver’s bare area, with care to protect the porta hepatis, gallbladder, IVC, and hepatic veins. The diaphragm’s peritoneum is then separated from its muscle. The chest is entered away from the disease to drain pleural effusion and access transdiaphragmatic tumor, which is resected along with the involved diaphragm. Repair is covered in a later video.
