Video Summary
Type B radical hysterectomy, also called modified or proximal radical hysterectomy, is used for early cervical cancer. After opening the vesicouterine space, part of the vesicouterine ligaments is removed and the ureter is unroofed. The paracervix is divided at the ureteral tunnel; in type B2, lateral paracervical lymph nodes are also removed. The deep uterine vein and dorsal paracervix are transected while protecting the ureter and pelvic nerves. At least 10 mm of vagina is resected. Preserving connective tissue lateral to the ureter may reduce fistula and stenosis risk.
