Video Summary
The joint Society of Abdominal Radiology–IGCS session explored how integrated imaging can guide ovarian cancer diagnosis and management.

For adnexal lesions, speakers emphasized using standardized O-RADS terminology rather than vague descriptions such as “complex.” Ultrasound is the primary assessment tool; MRI can clarify tissue type and determine whether a suspected solid component truly enhances. O-RADS 1–2 lesions are overwhelmingly benign, while O-RADS 3 remains a challenging gray zone requiring individualized assessment. Accurate characterization can reduce unnecessary surgery while helping ensure suspected cancers are managed by gynecologic oncology teams.

For advanced ovarian cancer, imaging helps determine whether primary cytoreductive surgery is feasible or whether neoadjuvant chemotherapy is more appropriate. CT is the mainstay for mapping disease, while MRI can help resolve specific questions about pelvic, bowel, or peritoneal involvement, and PET-CT is generally reserved for selected cases, particularly suspected recurrence. Imaging must assess not only disease volume but also its distribution and relationship to critical structures. Small-bowel and mesenteric disease can be difficult to detect and may limit surgery despite reassuring scans.

Speakers advocated systematic image review and actionable, structured reports that describe disease sites, surgical complexity, and diagnostic confidence. They also stressed close collaboration between radiologists and clinical teams, and awareness of imaging’s limitations—including both missed subtle disease and false-positive findings.
