Ed’s List highlights September 2026 research in gynecologic oncology, with ovarian cancer prominent across clinical care, prevention, and translational science.<br /><br />In advanced BRCA-wild-type ovarian cancer, the phase 3 KEYLYNK-001 trial found that pembrolizumab plus chemotherapy followed by pembrolizumab–olaparib maintenance improved progression-free survival versus chemotherapy alone, although adverse events were more frequent. Five-year KEYNOTE-826 results confirmed sustained survival benefits from pembrolizumab plus chemotherapy for recurrent or metastatic cervical cancer.<br /><br />A large BRCA-carrier study estimated substantial residual ovarian cancer risk when preventive bilateral salpingo-oophorectomy is delayed, supporting individualized counseling about surgery timing. By contrast, a multicancer blood-screening trial did not reduce the combined incidence of stage III–IV cancers after three rounds, so population-wide use remains unproven. A policy review likewise urges cautious, context-specific expansion of genetic susceptibility testing, given uncertain clinical utility for many genes and risk scores.<br /><br />Other treatment findings include evidence that greater hospital-level use of neoadjuvant chemotherapy was not associated with better or worse 10-year survival, and a retrospective study linking systematic lymphadenectomy at interval surgery with improved survival, alongside more complications. A review of colorectal cancer ovarian metastases supports considering complete surgical resection, since ovarian lesions may respond less well to chemotherapy.<br /><br />Basic and translational studies identify potential avenues to address ovarian cancer resistance and immune suppression, including engineered extracellular vesicles, USP15 inhibition, ctDNA response monitoring, immune-cell therapies, and strategies targeting stromal–immune interactions. Additional work explores fallopian-tube precursor lesions, tumor biomarkers, and ovarian cancer risk associated with PALB2 variants. Overall, the collection emphasizes promising advances while underscoring the need for stronger evidence before adopting emerging tests and treatments broadly.
