Video Summary
The IGCS–Society of Abdominal Radiology session focused on integrating imaging with clinical decision-making in ovarian cancer.

For adnexal lesions, speakers highlighted O-RADS ultrasound and MRI as standardized tools for estimating malignancy risk and guiding follow-up, further imaging, or referral to gynecologic oncology. Physiologic findings and classic benign lesions can often be recognized confidently, while indeterminate O-RADS 3 lesions remain a clinical challenge. Ultrasound is the primary assessment; MRI can clarify whether suspected solid components are enhancing tumor or benign debris and can improve specificity.

In advanced ovarian cancer, imaging helps assess disease distribution, surgical resectability, and operative complexity. CT is the main staging modality, with MRI and PET-CT used selectively for problem-solving or suspected recurrence. Reports should systematically describe sites that may affect surgery, including mesenteric and small-bowel involvement, upper-abdominal disease, pelvic organ interfaces, and distant metastases. However, imaging can underestimate small or diffuse peritoneal and bowel-serosal disease, so findings must be interpreted with these limitations in mind.

The speakers emphasized actionable, structured reporting and close collaboration between radiologists and gynecologic oncologists. A shared understanding of treatment pathways and imaging limits can improve triage, avoid unnecessary procedures, and support appropriate surgical planning.
