A 58-year-old postmenopausal woman presented with bleeding and discharge from the vagina, as well as lower backache. Physical examination revealed a cauliflower-like growth on the cervix, involvement of the upper third of the vagina, and medial involvement of the bilateral parametrium. Laboratory results showed no abnormalities, and imaging revealed a large mass in the cervix with nodal involvement. A biopsy confirmed moderately differentiated squamous cell carcinoma. The final diagnosis was stage III C2 squamous cell carcinoma of the cervix. The patient underwent neoadjuvant chemotherapy and showed a positive response. The treatment plan included external beam radiation therapy and brachytherapy. The radiation therapy planning involved defining the target areas, including the primary tumor and nodal targets, and contouring the organs at risk. The treatment was delivered using VMAT technique with specific dose prescriptions. Brachytherapy was performed using CT-based HDR intracavitary brachytherapy. The patient completed treatment and was currently on follow-up. The calculated biological effective dose (BED) for the tumor, rectum, and bladder were provided.
