This case demonstrates the surgical decompression of a giant 53 cm ovarian mucinous tumor in a 43-year-old woman to enable removal through a small abdominal incision. After mini-laparotomy, a pursestring suture was placed around a small cyst incision to aspirate 16 liters of fluid safely, preventing spillage. The collapsed tumor was then removed via left salpingo-oophorectomy and total abdominal hysterectomy with right salpingo-oophorectomy. The procedure, lasting 20 minutes for decompression, was well-tolerated with 500 cc blood loss and no transfusions. The patient recovered without complications and was discharged improved on postoperative day three.
