Video Summary
This video demonstrates a safe, feasible laparoscopic pronoposition approach for posterior inferior mediastinal lymphadenectomy in a 46-year-old woman with recurrent gynecologic poorly differentiated carcinoma. After prior pelvic lymphadenectomy and hysterectomy, a mediastinal lymph node recurrence near the thoracic aorta was partially chemo-responsive. Using selective ventilation and right-side 4-trocar access, the tumor was completely resected while preserving key structures like the azagous vein and intercostal branches. Surgical time was 196 minutes with minimal blood loss. The patient recovered well. This technique offers a standardized method for thoracic surgeons addressing posterior mediastinal tumors, though further studies are needed.
