A 43-year-old woman with a giant 53 cm ovarian mucinous tumor underwent mini laparotomy with a small infraumbilical incision. A purse-string suture was placed on the cyst to allow controlled decompression by aspirating 16 liters of fluid, preventing spillage. The tumor size was reduced for safe removal. Subsequently, left salpingo-oophorectomy and total abdominal hysterectomy with right salpingo-oophorectomy were performed. The patient tolerated the surgery well, with 500 cc blood loss and no transfusions. She recovered without complications and was discharged on the third postoperative day in improved condition.
