Video Summary
This video presents a multidisciplinary, robotically assisted total pelvic exoneration in a 70-year-old woman with recurrent vulvar squamous cell carcinoma involving the anal sphincter. The stepwise surgical approach included posterior, lateral, and middle compartment dissections, resection of affected tissues, and omentum mobilization with perfusion assessment via IV ICG. Ureters were clipped, and a colostomy with chondrite formation was performed. The pelvic floor defect was covered using a gracilis flap. Postoperative recovery included treatment of a cardiac event. Eight months post-surgery, the patient is cancer-free with good stoma function and improved quality of life.
