Intraoperative use of indocyanine green and Firefly imaging ensured ureteral safety. The omentum was interposed between rectum and vagina to reinforce repair. The procedure had minimal blood loss and no complications. Postoperatively, the patient recovered well with no fistula recurrence at follow-up visits.
