This surgical film presents the radical resection of a 41-year-old patient's retroperitoneal malarian carcinoma. Initially responding to chemotherapy and chemoradiation, the tumor later progressed, invading major vessels and surrounding structures. A multidisciplinary team performed extensive surgery including tumor resection, arterial reconstruction using a femoral vein allograft, and ureteral reconstruction with jejunal interposition. The inferior vena cava was occluded and not reconstructed. Postoperative pathology confirmed poorly differentiated malarian adenocarcinoma with negative margins. Four months after surgery, the patient shows no disease evidence and good recovery without complications. The case highlights complex management of rare retroperitoneal tumors.
