Standard treatment involves radical excision and lymphadenectomy, which carries high morbidity and lymphedema risk. SLNB offers a safer alternative for selected patients. A 65-year-old woman with vulvar melanoma underwent radical vulvectomy and bilateral SLNB using preoperative technetium-99 lymphoscintigraphy and intraoperative ICG instead of isosulfan blue. The approach allowed successful real-time lymphatic mapping and node removal. Results confirmed metastases in sentinel nodes. SLNB with technetium-99 and ICG is feasible and may benefit high BMI patients; further research is needed.
