Silva Pattern in Cervical Adenocarcinoma: what pathologists see and what gynecologists need to know

This webinar, part of the IGCS–MEMAGO ECHO Tumor Board series, looked at the Silva pattern of invasion in HPV-associated cervical adenocarcinoma, where pathology and clinical care meet. Experts explained how the Silva system sorts tumors by risk, what pathologists look for under the microscope, and how these findings guide surgical and treatment decisions. An interactive case discussion followed.

Topics and Faculty:

  1. Silva Pattern in Cervical Adenocarcinoma: What Pathologists See and What Gynecologists Need to Know – Dr. Takako Kiyokawa, President-elect, ISGyP
  2. Interactive Case Discussion: Applying the Silva Pattern in Clinical Practice - Dr. Abir Khaddage

The webinar aimed to help participants:

  1. Describe the WHO classification of endocervical adenocarcinoma into HPV-associated and HPV-independent types.
  2. Recognise the features of Silva patterns A, B and C and their link to the risk of lymph node metastasis, recurrence and death.
  3. Explain why lymphovascular invasion matters, especially in pattern B, and how the two-tier (low-risk vs. high-risk) Silva model works.
  4. Understand the limits of biopsy, curettage and cone specimens in assessing the Silva pattern, and what this means for clinical and fertility-sparing decisions.
